Do You Qualify?
Before we get going, we need to make sure that you qualify for a medical marijuana card in the state of Pennsylvania. We encourage you to make an appointment with us if you suffer from any of the following conditions. We do not require a physician’s referral for an appointment, our doctor will review your medical condition in one visit or telehealth call and confirm your eligibility.
Glaucoma
HIV/AIDS
Huntington’s Disease
Inflammatory Bowel Disease
Amyotrophic Lateral Sclerosis
Autism
Cancer
Crohn’s Disease
Terminal illness with a medical prognosis of life expectancy of approximately one year or less if the illness runs its normal course
- Damage to the nervous tissue of the spinal cord with objective neurological indication of intractable spasticity
Epilepsy

Multiple Sclerosis
- Neuropathies
- Parkinson’s Disease
- Post-Traumatic Stress Disorder

Register on the State’s Patient Registry.
You MUST register with the state before you appointment. All you need is a valid driver’s license or Pennsylvania state ID card. Once you have it handy, visit: https://padohmmp.custhelp.com/app/adult-patient-registration on a computer (the website isn’t very usable on a mobile device) and fill out the information exactly as it appears on your identification. Use your keyboard’s “CAPS LOCK” for the best experience. When you’ve completed your registration, you’ll receive a Patient ID Number. Make sure to write it down or keep it somewhere safe, you’ll need it for your appointment. Once your registration is done, return here to continue onto Step #3.

Certification appointments with our doctor are $150, due the day of your appointment.
Fill Out Our Intake Form
Before your appointment, we’ll need some information from you in order to make the application process as smooth as possible. In Step #2 , you registered with the state and we asked you to set aside your Patient ID Number that you received from that registration. This is the step you’ll need it for, so make sure you have it (and your driver’s license or state ID). When you’re all ready, check your email for an email from us (it will be from ArfinnMed Portal – that’s our health record system) FYI: Be sure to check your Spam folder, sometimes it gets bumped over there! Follow the link in your email to access the form!
Complete Your Registration
You’ve seen the doctor (if you haven’t, come back to this page once you’ve had your appointment), now you just have to complete your registration with the state. To do that; Login to the state’s website (on a computer) using the ID and password you set up in Step #2. Complete the registration and pay for your medical marijuana card (it will cost you $50). Once you’ve completed the process, all you need to do is wait to receive your medical marijuana card in the mail.
