Do You Qualify for Medical Marijuana?
If you suffer from one of the following conditions, you may qualify for a medical marijuana card in the state of Pennsylvania. No referral is needed, just scroll down and make an appointment below. Bring your medical paperwork with your diagnosis, during your visit or Telehealth appointment and confirm your eligibility.

Terminal illness with a medical prognosis of life expectancy of approximately one year or less if the illness runs its normal course

#1 Register with the State of PA
You MUST first register with the state of PA to get a medical marijuana card.
Visit https://padohmmp.custhelp.com/app/adult-patient-registration and carefully fill out the form in all CAPS.
Once registered, you’ll receive a five or six digit Patient ID Number. Write that number down now! You’ll need it to fill out the Halo Intake Form later on.

#2 Book an Appointment with a Halo Doc
We offer In-Person or Telehealth appointments to get your care with convenience.
Click FIND A DATE AND TIME below to choose your appointment type and a date and time that work for you.
Certification appointments with our doctor are $175, due the day of your appointment.
#3 Fill Out the Halo Intake Form
Before your appointment, we’ll need some information from you in order to make the application process as smooth as possible. In Step #2 , you registered with the state and we asked you to set aside your Patient ID Number that you received from that registration. This is the step you’ll need it for, so make sure you have it (and your driver’s license or state ID). When you’re all ready, check your email for an email from us (it will be from ArfinnMed Portal – that’s our health record system) FYI: Be sure to check your Spam folder, sometimes it gets bumped over there! Follow the link in your email to access the form!
#4 Meet With Our Halo Doc
In-Person or Online. It’s up to you! We make it easy to meet with one of our licensed doctors so you can have all of your questions answered. We want you to have a stress-free and comfortable experience, and our friendly docs do just that!
#5 Complete Your PA State Registration
Finished with your Halo Doc Appointment? Great! Now you just have to finish your PA Registration!
To do that; Login to the state’s website (on a computer) using the ID and password you set up in Step #2.
Complete the registration and purchase your medical marijuana card (it will cost you $50). Then, all you need to do is wait to receive your medical marijuana card in the mail!
